Step 1: Check you are within the practice catchment Area

Step 2: Click this link to access our registration forms Register with a GP surgery - our registration process is now paper free and can be completed online.

Paper registration forms are available in the practice if needed.

Please note: You may be asked to bring your passport or other photographic ID and proof of address into the practice to verify your registration.

Step 3: Someone from the practice will contact you to arrange a new patient health check.

Step 4: Attend your new patient check.

It is important that you attend your appointment with the nurse for a new patient check. She will take a medical history from and enter any significant information such as allergies and medications onto your computer record.

Step 5: You are placed on the practice list. Once you have completed and signed the form and attended your new patient check, you will be registered with the practice.
